The Columns in Accident report form 24 Rule 115 of factories act are as follows:

• Name and Address of occupier, and
• Occupier’s Registration Number License Number, and also
• Address of Premises where accident happened, and
• Nature of industry, and
• Department, Shift house if any and exact Place where the accident happened, and
• Name of the injured person, and also
• Insurance Number, and
• Address of the injured person, and
• (a. Sex, b. Age (Last birthday), c. Occupation of injured person, d. Local office to which attached.), and also
• Date and hour of accident, and
• (a. Hour at which he started work on Day of Accident, b. Whether wages in full or part are payable to him for the day of his accident.), and
• Case of accident, and
• If caused by machinery, and
• Give name of machinery and part causing the accident, and
• State whether it was moved by mechanical power at the time, and
• State exactly what the injured person was doing at the time, and
• In your opinion was the injured person at the time of accident, and
• Acting in contravention of the provision of any law applicable to him, and
• Acting in contravention of any order given by or n behalf of occupier, and
• Without Instructions from his occupier acting, and
• If reply to clause 1, 2, 3. Of clause c is in affirmative state where the act was done for the purpose of and in connection with the occupiers trade of business, and
• In case the accident happened while traveling by availing of the transporter facility provided by the occupier. State whether, and also
• The Injured person was travelling a passenger to or from his work place.

• उद्योग की प्रकृति, और
• विभाग, शिफ्ट हाउस यदि कोई हो और सटीक जगह जहां दुर्घटना हुई हो, और
• घायल व्यक्ति का नाम, और
• बीमा संख्या, और
• घायल व्यक्ति का पता, और
• (a। सेक्स, बी। आयु (अंतिम जन्मदिन), सी। घायल व्यक्ति का कब्ज़ा, घ। स्थानीय कार्यालय जिसमें संलग्न है।) और
• दुर्घटना की तारीख और घंटे, और
• (एक घंटा, जिस पर उन्होंने दुर्घटना के दिन काम शुरू किया, ख। चाहे पूर्ण या भाग में मजदूरी उनके दुर्घटना के दिन के लिए देय हो।)।)
