Maternity Benefit Register form 10 Rule 12 (1)
The columns in Form 10 Maternity Benefit Register are as follows :
Name of the woman, and
Date of appointment, and
Department in which employed, and
Nature of work, and
Dates (with month and year) on which she is laid off and not employed, and
Total days employed, and
Date on which woman gives payment period, and
Date of birth of child, and
proof of pregnancy under Section 6 of the maternity benefit act, 1961 – Date of production, and
Date of production of proof of delivery, and
Where the maternity benefit delivery, the date on which is paid in advance before it is paid, and the amount thereof, and
Date of which subsequent payment of maternity benefit is made and the amount thereof, and
Where the medical bonus is paid, the date on which it is paid, and the amount thereof, and
Date on which wages on account of leave are paid, and the amount thereof, and also
Name of the person, and also
Remarks

फॉर्म 10 मातृत्व लाभ रजिस्टर के कॉलम इस प्रकार हैं:
महिला का नाम, और
नियुक्ति की तिथि, और
विभाग जिसमें कार्यरत हैं, और
काम की प्रकृति, और
तिथियां (महीने और वर्ष के साथ) जिस पर वह रखी गई है और नियोजित नहीं है, और
कुल कार्यरत दिन, और
दिनांक जिस पर महिला भुगतान अवधि देती है, और
बच्चे के जन्म की तारीख, और
मातृत्व लाभ अधिनियम, 1961 की धारा 6 के तहत गर्भावस्था का प्रमाण – उत्पादन की तारीख, और
प्रसव के प्रमाण के उत्पादन की तारीख, और
जहां मातृत्व लाभ वितरण, जिस तारीख को भुगतान करने से पहले अग्रिम भुगतान किया जाता है, और उसके बाद की राशि, और
तिथि जिसमें बाद में मातृत्व लाभ का भुगतान किया जाता है और उसकी राशि, और
जहां चिकित्सा बोनस का भुगतान किया जाता है, जिस तारीख को उसे भुगतान किया जाता है, और उसके बाद की राशि, और
तारीख जिस पर छुट्टी के समय मजदूरी का भुगतान किया जाता है, और उसके बाद की राशि और भी
